Tribal Enrollment staff will be at Chinook Winds Casino on Saturday, November 19th during the Restoration Pow-Wow.

They will be providing:

• Tribal ID cards

• Address & Contact Information Update forms

• Name Change forms

• Designation of Death Benefit Beneficiary forms (you must have form notarized before returning)

• Application for Enrollment
