Regional Community Health Survey Opens

Survey

Lincoln County Public Health is seeking community input for the next Regional Health Assessment (RHA). Those who live, work, or play in Lincoln, Linn, or Benton counties are invited to complete a brief, anonymous survey, which will help inform future community health planning.

The RHA is a partnership between InterCommunity Health Network Coordinated Care Organization, Samaritan Health Services hospitals, Confederated Tribes of Siletz Indians, United Way of Linn, Benton & Lincoln Counties, and the local public health departments of Lincoln, Linn and Benton counties.

The Public Health Department conducts a Community Health Assessment every three to five years. The last Lincoln County Community Health Assessment was released in 2018 and the last Regional Health Assessment was released in 2015. Both assessments can be found on the county website. These assessments provide and explain a wide range of information about health factors and indicators that influence health in Lincoln County. They also inform the Public Health Department’s work and provide local and regional partners with up-to-date health-related data.

The Regional Community Health Survey can be accessed online in English or Spanish at the links provided below:

Paper copies of the survey can be found at the public libraries in Lincoln City, Newport, Toledo, Siletz and Waldport. For surveys in additional languages, please contact Madelyn Hiner. The last date to complete a paper survey is June 13th  and the online survey closes June 17th.
